STEP 1: DEALBREAKERS *

While we celebrate diverse opinions, backgrounds, and lifestyle choices, the following requirements are all deal breakers. If you can't check yes for all of these boxes, you do not need to submit your application as you will not qualify for an apprenticeship at our studio.

I understand the following information is provided of my own free will and is accurate to the best of my knowledge.

I am at least 18-years of age.

I am fully vaccinated against COVID-19, including any boosters after 6-months from my last dose, OR I am willing to get fully vaccinated and maintain a fully vaccinated status for the duration of my time with Black Candle Tattoo Co (including supplementary boosters.)

I am willing to wear a mask when I tattoo. Additionally, I'm willing to wear a mask anywhere in studio when required by the studios health policy.

I have received and am up to date with vaccinations for Tdap, Polio, MMR, & Hepatitis B, OR I am willing to get these vaccines before starting at Black Candle Tattoo Co.

I am willing to get the seasonal Flu Vaccine, once a year, as required by the Black Candle Tattoo Co health policy.
